Position Overview:
Under the direction of the Primary Care Manager, the Clinical Registered Practical Nurse is responsible for Conducting preliminary visit interview for patients visiting Physician/Nurse Practitioner, Communicate with clients and external providers and series regarding the tracking of test results, follow up appointments, written prescriptions and triage urgent phone calls.

- A minimum Registered Practical Nurse Diploma is required.
- A member in good standing with the College of Nurses of Ontario.
- A member of Registered Practical Nurse Association of Ontario (RPNO).
- Certification in Foot Care is an asset.
- Three years work experience in a collaborative care model; (Family Health Team) is preferred.
- Experience working with Anishinabek people is an asset.
- Candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications will also be considered.
- Satisfactory Criminal Records Search and Vulnerable Sector Search.
- must possess a valid Standard First Aid/CPR Certificate or be willing to obtain one;
- ability to work flexible hours as determined by the Service Manager or designate in relation to service delivery requirements;
- must posses a valid Ontario Driver’s License and access to a personal vehicle.
- must provide evidence of registration with the appropriate regulatory bodies (College of Nurse and Registered Nurses Association of Ontario)
